Phantom Breast Syndrome: Experience from a UniversityHospital in a Developing Country by

The aim of this study to assess the presence and the significance of phantom breast syndrome (PBS) in a conservative community with the cultural stigma of emotion disclosure related to mastectomy.This study was conducted at the breast oncology clinics between Jan 2017-December 2018. Direct interview using a prepared pilot tested structured questionnaire directed towards patients who underwent mastectomy between the periods of January 2011-December 2017. Pain and body image distortion were the focus of the study. Demographic data, marital status, education and socioeconomic status were included. The target population intended was 100 patients 347 patients were diagnosed with breast cancer, mastectomy was performed on 143 (41.2%). The capture rate for interview was 82 (82%) of the targeted interviewees. Age ranged between 23-83years with the median age of 49 years. The majority of women who experienced phantom breast syndrome 30 (36.5 %) were within the reproductive age group. Progression of pain after the first year of mastectomy was noticeable ranging from phantom pain to severe pain requiring aid with standard analgesics with successful relief in 57 (69.5%) of the patients. Body image distortion concerns and phantom pain symptoms were more pronounced in married 29 (35%), highly educated 32 (39%) and high socioeconomic class 13 (16%) compared to the rest of the group. It was also noticed that pain symptoms and severity decrease with age. PBS remains a significant entity that should be identified, investigated and treated in order to contribute to high quality of life in these subsets of women with breast cancer. Special tailoring to own cultures may aid in rehabilitation of the affected women.

Mammarokoronarny Shunting on the Working Heartby the “Midcab» Method and Gene Status: The FirstExperience in Uzbekistan by Alexander Nagay in Surgery & Case Studies Open Access Journal in Lupinepublishers


Surgical methods are often applied in treatment of patients with coronary heart disease. The modern direction in development of coronary surgery today became the transition to mini-accesses and to an arterial revascularization. For lifetime of the coronary shunting (CS) enough experience was accumulated of its performance in the conditions of artificial blood circulation. Material and Methods: Minimally Invasive Direct Coronary Artery Bypass MIDCAB. The patient arrived with the diagnosis: Coronary heart disease, stenocardia. EH-III stage, risk 4 very high. A concrement in the right kidney, heart failure IIA stage. Functional class- III NYHA. Genotyping of the SNP was performed by polymerase chain reaction Real-Time PCR. (essential hypertension panel (9 genes); Coronary heart disease panel (8 genes)).
The operation by a technique of “MIDCAB” was executed in our center in September 2017, for the first time in Uzbekistan. The patient had a high genetic risk of predisposition to hypertension and thrombophilia. Surgery was carried out in conditions - high chest epidural anesthesia in combination with an endo- tracheal anesthesia. Artificial ventilation of lungs was carried out through an intubation tube in the two-pulmonary mode of ventilation. The patient was stacked on a back with a body tilt 15-30°. Access to heart was made through a left-side front side thoracotomy in the fifth midrib. Length of a skin section was 12 cm. Owing to small experience, a section was made long - 12 cm. Surgery time, also was long. Now performance of MIDCAB is carried out in 135 min. Blood losses are also reduced. Gradual understanding of advantages MIDCAB operation by most of surgeons, led to a revascularization alternative through a sternotomy. Thus, using MIDCAB technology is the effective strategy of treatment patients with coronary heart disease. It allows to reduce injury of surgery, to improve the postoperative period, to reduce quantity of complications. However, there are still many surgeons who skeptical about this operation, because of fears about the early and remote postoperative results. The clinical picture coincided with the genetic status of the patient. It is worth noting that the disease has variable expressivity and penetration, with the presence of additional genetic and environmental factors being important in many cases.

Complications of Stoma and The Management by Thamilselvam P in Surgery & Case Studies Open Access Journal in Lupinepublishers

A patient with stoma (colostomy or ileostomy) has poor quality of life and when he/she faced with stomal complications (Skin eczema, ischemia/necrosis, retraction, mucocutaneous separation, parastomal abscess, colostomy diarrhoea, parastomal hernia, obstruction, prolapse and retraction), the quality of life would go further down. So the people who are working in health system need to have more knowledge about the stoma and its complications. The definition of Quality of life is individuals’ perception of their position in life in the contest of the culture and value system in which they live, in relation to their goals, expectations, standards and concerns affecting complex ways by the person’s physical health, psychological state, level of independence, social relationships to salient features of their environment [1].The quality of life is very important problem for the patients with colostomy since they underwent surgery which made them to travel in the society with a new quality of life [2].
