Acute Liver Failure and Thyrotoxicosis Managed with Liver Transplant and Thyroidectomy by Gerardo Tamayo-Enriquez in surgery and case studies open access journals in lupine publishers

Acute liver failure and hyperthyroidism are not typically common, although some cases have been reported. The mechanisms involved and optimal management are not well-defined. This article presents the case of a 32-year-old African American female referred for evaluation of abdominal pain and jaundice, with a past medical history of systemic lupus erythematosus and Grave’s disease. She had thyrotoxicosis after administration of contrast and developed acute liver failure culminating in liver transplant and subsequent total thyroidectomy with a favorable outcome.

Medical Case Study Journal - Lupine publishers


Studies have shown incidence of 1-5 % SCC development in old psoriatic plaques out of which sacrum is rare site. In our case we excised a post psoriatic SCC and reconstructed it with a gluteus maximus turnover flap on one side along with fasciocutaneous only VY advancement flap on other side.A 45-year-old gentleman was presented with an ulceroproliferative growth on sacral region over the pre-existing lesions of psoriasis for the last 6 months. The lesions were firm to hard in consistency, fixed to underlying fascia and muscle, bone was free, there was sero-sanguineous oozing from the lesions. Histopathology was suggestive of well-differentiated squamous cell carcinoma (SCC). Lesion was excised with 2 cm margin, in depth the level of excision was till periosteum of sacrum. The true soft tissues defect was around 15 cm by 10 cm with exposed sacrum in the middle part. On right side gluteus maximum muscle only turnover flap was raised, and on left side VY faciocutaneous flap was raised to cover the defect. Patient was discharged on 10th Post-operative day.We conclude that long standing psoriatic ulcer should always be assessed for conversion into malignancy. Gluteal maximus muscle flaps can be used with various combinations with fasciocutaneous flaps for reconstruction of sacral defects.

There is currently no validated micro(mi)RNA diagnostic stool test to screen for colon cancer (CC) on the market because of the complexity of fecal density, vulnerability of stool to daily changes, and the presence of three sources of miRNAs in stool (cell-free from fecal homogenates, exsosomal miRNAs from fecal exosomes, and fecal colonocytes). By employing earlier on a microarray miRNA experiment, using Affymetrix GeneChip miRNA 2.0 Arrays, on immunocaptured and enriched stool colonocytes of 15 subjects [three healthy controls and twelve colon cancer patients [three TNM stage 0-1 (e.g., polyps  1 cm, villous or tubvillous, or with high grade dysplasia), three stage 2, three stage 3, and three stage 4] in triplicates, this allowed for selection of a smaller panel of 14 preferentially expressed mature miRNAs associated with colon cancer (12 Up-Regulated, miR-19a, miR-20a, miR-21, miR-31, miR-34a, miR-96, miR-106a, miR-133a, miR-135b, miR-206, miR-224 and miR-302; and 2 Down-Regulated, miR-143 and miR-145). Then carrying out an absolute quantitative digital PCR on these 15 stool samples from TNM stages 0-4 on total small RNA extracted by immunocapture, followed by RT that employed a Custom TaqMan® miRNA Reverse Transcription (RT) Kit and TaqMan RT Primer Pool, and absolute quantification of miRNAs, in copies/μl, measured using a chip-based Absolute QuantStudio 3D Digital PCR analysis, allowed for validating the microarray results. To ensure that human and not bacterial small total RNA was chosen, coextraction protocols with E. coli K1 strain RS18 was carried out, followed by comparing Agilent electrophoretic patterns with human and bacterial electrophoretic patterns, and also random samples were sequenced using mRNA/miRNA sequencing, to ensure that human and not bacterial mRNA was chosen.

Quality of health service is measured based on structure, process and outcome elements and has multidimensional concept that has been defined in various ways. It is an important aspect of health service delivery system and must be given priority in most countries.A facility based cross sectional quantitative and qualitative study design was employed from January- May 2016 on 396 guardians of under five children. The data were collected using structured exit interviews, facility assessment checklists and record reviews. Data was cleaned and entered Microsoft office Access, EPI info and analyzed by Statistical Package for the Social Science. Binary and multivariate logistic regression was used to identify factors of guardians’ satisfaction and odds ratios, 95% CI and p-value was computed to measure the presence and strength of associations.The study showed that health centers and health posts on average fulfilled 32.4% of the health human power need and 44.1% of major equipment requirement against the national standards. The average adherence of health professional with guidelines of pneumonia and diarrhea managements was 76% and 65.6% respectively. Malnutrition was inappropriately managed and alarming in all components which needs program interventions. The composite average satisfaction level of guardians was 54.3%. Adequacy of information given about child health service (OR=0.49, 95% CI, =0.27, 0.91), expectation about the health services (OR=0.46, 95% CI, = 0.23, 0.91), privacy given during treatments (OR=0.21, 95% CI, = 0.08, 0.54) and preference of health institutions were the predictor variables (OR=0.18, 95% CI, = 0.09, 0.35) for under five child health service satisfactions of guardians.The overall quality of health service was low compared to studies done at different areas on quality of health care. The health institutions were facing shortages of human power, material resources and low adherence with standard treatment guidelines especially on diarrhea and malnutrition managements. The responsible bodies of health sectors at different levels should take actions to improve institutional capacity and performance of care providers in order to improve quality of health services. There should also be continuous monitoring systems to health facilities focused on health care services to strengthen institutional capacity and performance of care providers.

Management of proximal and distal tibial fractures is challenging because of the complexity of injury, limited muscle coverage and poor vacularity. Surgical management of tibial fractures includes multiple options: external fixation, IM nailing, ORIF and minimally invasive plate osteosynthesis (MIPO). Open reduction and internal fixation with a plate may end in wide dissection and tissue devascularization. Fixation of tibial fractures with MIPO allows protection of soft tissue and blood supply. This is a report of a series of prospectively studied closed proximal and distal tibial fractures treated with MIPO.A total of 20 patients with closed proximal or distal tibial or fractures were enrolled in the study between 2016 and 2017 and completed follow-up. Demographic characteristics, mechanism of injury, time required for union, range of motion and complications were recorded. Eleven patients had proximal tibial fractures and nine had distal tibial fractures. Patients were followed up to 2 and 6 weeks and then at intervals of 4 to 6 weeks until 12 months.Mean age of the patients was 39.5 years (range 18-65 years). Thirteen cases were the consequence of high-energy trauma and seven were the result of low-energy trauma. The clinical and radiological outcome were excellent in 13 patients, good in 6 patients and fair in 1 patient. The average time for fracture union was 16.4 weeks, ranging from 16 to 20 weeks. None of our patients developed any complications.MIPO is a reliable method of management for tibial fractures; it provides a high union rate and good functional outcome with minimal soft tissue problems.
